Lost alcohol awareness cards
If you lost your alcohol awareness card you may be able to replace it without reattending a class.

Your first step is to contact the provider who issued your card. If you do not know which provider issued your card contact your human resourses office or manager and ask them. There is no longer a central database and there are numerous providers. Providers only have access to their own records.

Information that may be asked is your name, card number, date or approximate date of issue and birthdate.

If you obtained an ACT alcohol awareness card from Nevada Alcoholic Beverage Control you may be able to obtain a replacement card online. Check the ACT database first.

If you have a valid Alcohol Awareness card you can then go to our Lost Card link to pay for the card. You may be require to set up an acount first. There is no charge to set up this account. You will receive a PDF copy of your card. Your card will then be sent out with in 2-5 days.

If you obtained a card from another provider please download the provider list. If your provider is not on the list the you will have to attend another class. You can a class online.
